New Students: Six Week Screener

All new students are evaluated after six weeks on basic well being and learning. When I used to teach in Arizona, there was a 40 day screener that all new students were evaluated on, as per state mandate. When I went international, nothing to check up on new students was in place. I presented to the school leadership and all supported this initiative.

Our school has a Student Resource Team (SRT, similar to a student study team, to identify students at risk) and a formal referral process, but nothing was bridging the gap for teachers. Teachers tend to not refer a student unless they have a STRONG feeling, but this is much easier to complete, and is survey style vs. written paragraphs.

This form collects any concerns about the following:

Hearing and Vision

Communication

Academic

Self Care

Social

Behavioral

Motor Skills

We are a 1:1 school using Google for almost everything. Making the decision to use Google Forms was simple. I mean why not? It does everything for you once a submission is made.

Once all teachers have made the submissions, any potential problems are brought up at our school SRT meeting. This can help make the referral process go quicker and is another piece of data we can use if any difficult discussions need to be made.
