Weekly Student Wellness Survey

The PE teacher and I have been collaborating in an amazing way. She came up with it and now I'm sitting back thinking "Why didn't I think of this before?"

Each week she is starting her class with a general wellness survey. The results are sent to the counselors as well as herself.

She has the students rate their:

  • happiness/contentment on a 0-10 scale
  • Stress on a 0-10 scale
  • Has them choose what category their major stressor is for the week
  • If they ate breakfast
  • If they finished their homework
  • Their bedtime
  • The hours of recreational screen time
  • The hours outdoor activity time
  • If they would like to meet the counselor - there are 3 options here - no, Yes - I would like to talk but I can wait until my counselor has some free time, and Yes - I would like to talk as soon as possible

These questions are wonderful for many reasons. First I get to see immediately if someone would like to come see me. Students aren't always proactive in coming so reaching out in a safe environment and asking them is great for my students.

This also provides us with much more comprehensive student data if there is ever an issue. Responses are dated so we can look at patterns.

I also pull students in who's happiness/contentment are low, even if they say they don't want to meet with me, and if their stress is high. Even if the students responded that they didn't want to meet with me, I let them know how concerned I am about them and that's why I wanted to meet with them.

In the past month she has implemented this I have doubled my face time with students, which I am ok with. It has also helped the students know that we care about them and take their wellness seriously.
At my next position I will for sure pair up with a PE or health teacher to implement this!
